  • Type all names using Microsoft Word, Word Perfect or
    other word processor exactly the way you want them to appear. Check your spelling.
    We are not responsible for spelling errors on information provided by the customer.
  • Do NOT use a spreadsheet or Excel file
  • Do NOT type or format into columns
  • Type the word "and" Do not use "&"
  • Type out each state name in full
  • Type a continous list leaving 1 space between lines
  • Save as an "ASCII" file or "TXT" file only on a PC
